About the facility
Our major sites include University Hospital Geelong in Geelong's CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential Aged Care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
Barwon SouthWest Aged Care Assessment Service (BSW ACAS) provides comprehensive assessments to older persons in the Barwon South Western Region to assist them to identify their care and needs and gain access to appropriate care. These include approvals for Commonwealth Government subsidised care. This is a multidisciplinary service.
About the role
Are you looking for a rewarding career that will make a difference to the aging population? As a Team Leader and Assessment Clinician your role will be to conduct a Comprehensive Assessment to assist the consumer with choice of appropriate service and information to meet their needs and preferences. You will also provide daily support to the team to ensure the operational needs are met. This may include facilitating access to Residential Care, Residential Respite, Home care Package or broader community services for example: Regional Assessment Service, Mental Health, NDIS and CHSP.
