An exciting opportunity for an experienced Case Manager to join our Permanency Support Program (PSP)
About the roleYou will provide specialist case work and therapeutic intervention for children and young people, their families, and carers within the permanency support program using contemporary practices.
You will be responsible for:
- Developing and implementing case plans for children and young people, and providing ongoing therapeutic intervention and case management
- Assisting in the transition of children and young people entering, and exiting permanency support program care arrangements
- Engaging with children, young people, families and carers, and undertake assessments to identify their support needs
- Adhering to mandated reporting obligations and legislative requirements
About you
- A minimum of a Diploma in relevant discipline such as Community Services. Preference bachelor’s degree in teaching, Social Science, Nursing, Counselling
- Willingness to participate in an agency on call roster
- Experience working with and providing quality assessment and case management services to at risk families or children/young people in out-of-home care and carers
- Demonstrated understanding of the principles of contemporary case management practice and trauma informed practice
- CDP Certificate from DCJ, and / or Knowledge of the Child Protection legislation that underpins the work of a Case Manager in Out of Home Care including knowledge of the Office of the Children’s Guardian NSW Child Safe Standards for Permanent Care and the Children & Young Person (Care and Protection Act) 1998
- Current Working with Children Check clearance and Nationally Coordinated Criminal History Check (NCCHC)
- Current and valid driver’s licence with access to a well-maintained vehicle which will be used for work purposes
