This is your opportunity to be part of a team that actively contributes to patient safety by following policies, procedures and protocols of the North West Hospital and Health Service. Engage with the Aboriginal and Torres Strait Islander patients and their families enabling them to become informed about their diseases and the options for care available to them within NWHHS. Support continued improvement of health services by always participating in quality improvement opportunities. Work in partnership with other providers of primary health services for clients with chronic health disease conditions, both within NWHHS and with external providers.
The Advanced Health Worker reports to the Nurse Unit Manager Nurse Navigator & Respiratory Service.

Base salary ranges between $89,598 and $94,514 p.a. (HW6)
Requirements

• Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care is mandatory for appointment at the HW6 level.
• Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
• Travel to work at various facilities including remote sites within the NWHHS, by air or road travel, including day trips and/or extended trips over several days is a requirement of this role and a C Class Motor Vehicle Licence is required for operation of fleet vehicles.
• Pre-employment screening: working with children (Blue Card) and criminal history checks.
• This is a Vaccine Preventable Diseases (VPD) risk role and may require evidence of vaccination or immunity to: Hepatitis B, Measles, Mumps, Rubella, Varicella, Pertussis, Tuberculosis and COVID-19.
• A 3-month period of probation may apply.
