Job description

Hospital in the Home (HITH), Metro South Addiction and Mental Health Services are looking for an Advanced Peer Worker to join their team. This role aims to inspire and support consumers through formal peer support, practical assistance, tailored education, and referrals; helping them build resilience and engage with their community while providing education and information from personal recovery experiences. Share personal experiences of mental health issues to model recovery strategies, aiding in the assessment, intervention, and referral of Mental Health -HITH program participants.

About the Role

  • Staff are expected to be compliant with the timely and accurate input and collection of consumer related demographic information, diagnosis, outcomes collections and provision of service activity into appropriate applications. The data and information include the electronic entry and completion of documentation to meet legislative requirements, including Mental Health Act 2016 documentation, admissions, discharges and transfers.
  • Be an effective and collaborative member of the multidisciplinary team, and work within professional boundaries in accordance with peer work values to provide safe and quality practices as part of an integrated model of care.
  • Utilise experience, group facilitation skills and knowledge in consumer education and support programs, focusing on peer-led education and provide a responsive framework that meets the needs of consumers.
  • Actively participate in regular supervision practices (operational and professional) and performance coaching and development processes.
