About the Role:

An exciting opportunity has become available for an experienced oncology / haematology nurse to develop their skills in the role of Cancer Care Navigator at Joondalup Health Campus.

  • A dedicated Cancer Care Navigator will be with patients and their families on every step of their cancer journey, to coordinate all aspects of care and provide invaluable emotional support.
  • You will be responsible for applying advanced knowledge and skills to facilitate the continuity and quality of care of patients diagnosed with cancer. Through coordinating, assessment and monitoring of a comprehensive care plan for patients from their diagnosis, through treatment to discharge and follow-up or palliative care.
  • You will act as a liaison and resource to all members of the multi-disciplinary team within the hospital regarding strategies for quality Cancer Care management.
  • The Cancer Care Navigator will work closely with the team treating each patient, and provide a holistic approach to care by guiding patients towards services that may help.
  • Through attendance at relative multi-disciplinary team meetings you will be able to promote and guide a multi-disciplinary team approach to decision making in relation to patient care.

What we are looking for:

The Cancer Care Navigator will need to have:

A strong clinical skillset with extensive experience in the oncology / haematology specialty. Critical for this role is your ability to supervise, lead, direct and motivate others to achieve defined outcomes. Well-developed planning, prioritisation and time management skills are essential for this role.

You will also bring:

  • Relevant registration with AHPRA
  • Minimum of five years post registration in speciality area of Oncology / Haematology nursing
  • Understanding of scope of practice in line with relevant registration
  • Demonstrated experience in continuous improvement and initiatives supporting the customer experience
  • Demonstrated ability to work effectively in a multidisciplinary team environment and independently as required
  • Highly effective communication and interpersonal skills
  • Proven project management skills with sound organisational and change management skills
  • Computer literacy.

Desirable

  • Possession of or significant progression towards the attainment of a post graduate qualification in the area of specialty.
  • Understanding of health fund requirements for Cancer Care services.
