You will be working in the community, and helping clients remain as independent as possible in their own homes.

Requirements:

• Significant experience within the Aged Care industry (1-2 years minimum)
• Cert 3 in aged care/Alternative qualification 
• Significant case management experience required 
• Understanding of the Aged Care Sector as well as Home Care Package and other programme guidelines and service requirements
• The ability to build strong relationships and networks within the community
• Current Australian Drivers Licence
