In the last 14 months of operations, attend support have been extremely lucky and have expanded more than we could have imagined. Whilst growing, our main goal has always been to remain grounded & focused on providing services that are trustworthy & foster strong, long term relationships.
We are now in the stages where we need another Care Coordinator to join our team & come along with us in creating something special. The right individual will be caring, motivated, responsible and dedicated to understanding each participant they support.
About the role 🫶
We employ Care Coordinators within attend to help manage needs and supports. The role has many different aspects but overall the main theme is managing the service delivery for a number of our participants and being their point of contact.
Role & Responsibilities 💛
- Onboarding new participants - meeting with them to discuss their needs, learning about their interests and the type of support worker that will be beneficial for their team. In this aspect of the job, your compassion and understanding personality will be an asset. We don't like to pair just any worker with a participant, recognizing compatibility and dynamic are a huge part of our process.
- Discussing care needs and helping develop routines, schedules or processes to make sure supports are effective.
- Liasing with other aspects of supports - this may be family or other professionals.
- Mentoring Support Workers and implementing their service delivery - this could be attending a meet and greet with the new worker, dedicating time to their understanding of needs or organising professional development to prepare them.
- Communicating with Support Workers around participant needs.
- Checking “Case/Shift-Notes” and following up any changes.
- Regular catch ups in-home with participants (we like to do this monthly as a minimum).
- Basic rostering and updating care plans when needs change.
- Being point of contact with the participants via phone or email.
- The most important aspect of this role is being able to understand the participant and build a trusting relationship with them and their team of supports.
Experience & Requirements 💜
- Background of working within Disability or NDIS
- Understanding of NDIS plans, line items & implementing supports
- Strong communication skills
- Desire to create change and foster relationships within your role
- Kind and nurturing nature
- Ability to support with basic admin tasks where needed
- Relevant education or experience in disability
We work with a very small case load of no more than 10 participants to each Care Coordinator. This is our magic number - we keep this load low to ensure you are able to focus on your role without being overwhelmed, overworked and unable to deliver a high level of care to supports. Our team is small, our office is calm - and we are truly dedicated to making sure every one of our employees are doing something that they enjoy.
