Position Overview: Reporting to the Regional Manager, as the Care Manager, you'll play a crucial role in ensuring the highest standard of care for our clients while upholding our company values. Your responsibilities will include maintaining client portfolios, developing and evaluating care plans, and delivering excellent customer service.
Key Responsibilities:
- Provide holistic care to clients, focusing on their social, emotional, spiritual, and environmental needs.
- Develop and evaluate goal-based care plans in consultation with clients and healthcare professionals.
- Monitor and reassess care plans regularly, ensuring clients' evolving needs are met.
- Act as a client advocate and assist in accessing community resources and support services.
- Provide frontline management and supervision as needed.
- Effective, accurate and timely documentation in accordance with the requirements of the Aged Care Standards.
- Certificate III in Individual Support or similar qualification.
- Experience in Community Aged Care preferred.
- Strong interpersonal and communication skills.
- Ability to work autonomously and as part of a team.
- Proficiency in Microsoft Office applications.
