Care Service Coordinators - Gold Coast - HCP
We are seeking a Care Services Coordinator who is motivated, reliable, compassionate and organised for our HCP program. The Care Services Coordinator will based at our South Coast Office.
The full time position will be responsible for coordinating support services to eligible clients by using a team of in home care staff, clinical and allied health professionals, as well as brokerage services. This position includes $1,600.00 allowance with a fully covered vehicle with private use of up to 200km per week, a mobile phone, plus 11% superannuation and NFP salary packaging.
Overall the Care Services Coordinators' responsibility is to provide quality Care Services that are compliant with the agency's policy and procedures and the Age Care Quality Standards. The successful applicant will be working alongside other Coordinators and liaising with clients, family members and staff to ensure services are provided in accordance with the client's needs, goals and preferences.
Co.As.It. Community Services provides quality home support services to the general community and specialises in services to the CALDB (Culturally and Linguistically Diverse background) community in the Brisbane and South Coast areas. The agency has been responding to individual and daily needs of the aged in the community since 1978 and aims to always provide a service of excellence that is responsive to client's individual circumstances.
Requirements:
- Demonstrated knowledge and experience in a similar role for coordinating consumer directed Care Services to clients eligible for a home care package.
- Have advanced communication skills and the ability to model a positive culture and strong values through a professional service
- Have an understanding and experience in delivering services according to the Aged Care Quality Standards
- Previous experience coordinating similar services is desirable
- Demonstrated understanding of the changing and diverse needs of clients
- Proven relationship management with internal and external stakeholders, as well as good interpersonal and negotiation skills
- Previous client service coordination experience in the health/aged care not for profit, community services sector
- Ability to communicate in another language would be an advantage
- Tertiary Qualifications/Diploma in Community Services
- Current Driver licence
- Current National Police Certificate issued by the Australian Federal Police Canberra
- Full COVID-19 Vaccination Certificate
