About the Role:
NCN Health have an exciting opportunity for an Carer Support Coordinator to join our team at Numurkah.
The Carer Support Coordinator is responsible for the provision of information, education and resources, and supports the planning and coordination of flexible respite services to support local carers who provide unpaid care to a family member, friend or person who is frail aged, has dementia, a disability, mental illness or a medical condition.
The Carer Support Coordinator will assess the needs of the individual carers and work with them to plan and coordinate the provision of supports and services that meet their individual circumstances.
The NCN Health Carer Support Program provides person centred care and support via provision of flexible respite and support services for unpaid carers of people with care needs. Services aim to assist in meeting the diverse and individual needs of carers, promoting improved health and wellbeing of carers, and those for whom they care.
The Carer Support Coordinator is an integral part of the multidisciplinary Community Aged Care team.
What we are looking for:
- Vaccination against Covid-19 is a mandatory requirement
- Tertiary qualification in a health related discipline, including Nursing, Disability, Welfare and/or Community Services practice including current registration with relevant regulation agency.
- Excellent time management and organisational skills
- Demonstrated experience and well developed skills in the delivery of assessment, care planning and coordination of services for individuals and families with diverse needs.
