About the role
We are seeking a dedicated Case Managerfor our Homelessness Support Services! You will provide case management support outreach to individuals and families with multiple complex needs who are homeless or at risk of homelessness.

Responsibilities include

• Utilise a strengths-based, Client centred model of support
• Implement case management processes appropriate to needs of the Client
• Deliver case management processes with program and organisation models
• Assist Clients to access support services and advocate where required
• Maintain accurate Client records for all Clients and meet all funding body
• Actively participate in program Team Meetings and related activities

This is a permanent part-time position, 30.4 hours per week, based in Seymour, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.

What you will have

• Degree or diploma qualification in Social Work, Welfare, Community Development or related field OR
• High level of sector knowledge/skills attained through service & study/development
• Relevant experience in Social Service environment with focus on homelessness
• Experience in Client assessment
• Relevant experience using appropriate risk models to assess levels of risk, harm and well-being, including immediate risk
• Experience working effectively with people who exhibit multiple/complex service needs and/or are chronically disadvantaged
• Relevant experience with Client data and reporting systems, people management systems and Microsoft suite
• Current Driver's Licence (Australian State or Territory)
