We are currently on the hunt for a dedicated Case Manager to facilitate our unique group programs, ensuring our participants experience meaningful interactions and are supported to achieve their individual goals. You will be tasked with providing individualised support and services to our participants including weekly case management sessions.
About Us
Happy Paws Happy Hearts (HPHH) brings socially isolated people together in group sessions, to care for and train rescue animals. Our unique ‘twist’ on traditional animal therapy is that we bring participants to local rescue shelters and wildlife sanctuaries. Instead of taking trained pets to people, we’ve developed a more robust and scalable model which takes the people to the animals in need. It’s a win-win for two complex social challenges.

About The Role
In this role, you will be helping people experiencing social isolation on developing their job readiness skills, building their confidence, and achieving their goals. You will also be helping rescue animals, which participants help to train and care for during our programs to improve their chances of adoption.
To succeed in this role, you will have the following personal attributes:
- Demonstrated ability to work in a dynamic team and the ability to deliver on the Vision/Mission of Happy Paws Happy Hearts.
- Exceptional communication skills and the ability to work respectfully and collaboratively within a team environment.
- Demonstrated experience working with people with a disability and understanding of the principles of choice and control in services delivery.
- A passion for animal welfare and experience, either as a hobby or working, with a variety of different animals.
- Excellent organisational skills and time management skills, and ability to take initiative and prioritise workload and deadlines.
- Sound computer skills including demonstrated competence in the use of various business systems including CRMs and Microsoft Office.
- A sense of accountability and recognition of internal and external feedback (be it positive or negative) as an opportunity for growth.
- Ability to embrace change and continual improvement and play an active role in the expansion of service offerings.
- Demonstrated experience in leading a positive health and safety work culture.
The role is based in Dakabin. This is a casual position approximately 8-12 hours per week.
Qualifications & Experience
- Minimum Diploma Level Qualification in relevant field (Human Services/Social Work/Community Services /Allied Health)
- Proven experience, knowledge and / or training relevant for working with people who are experiencing social isolation, including people with disabilities, and people experiencing mental health challenges.
- Demonstrated communication skills and the ability to work respectfully and collaboratively within a team environment
- Current Working With Children Check and NDIS Worker Check or ability to obtain (essential).
