The Opportunity
Do you have a passion for supporting people at risk and want to make a difference within our community? Through a collaborative approach for services and support, you can make a difference to Adults and Families who are at risk or experiencing homelessness.
Our Case Workers take a client centred approach to identify goals and service needs to increase capabilities and promote an improved quality of life. The role includes provision of information, referral, advocacy support and linking participants to services and ensuring access to secure housing options.
Case Workers play an important role in empowering our clients to lead the life they want to live. Adults and families often present with complex needs, such as mental health concerns, trauma, substance use issues and family and domestic violence. This role provides the opportunity to make a difference in a vulnerable community members life and support them to transition to safe and secure long-term accommodation.

Duties
- Function within relevant policy, procedure, legislation, industry requirements and guides and health and safety requirements.
- Actively contribute to a positive team culture with a solution focused, client centred approach.
- Participate in relevant training, professional development, and performance reviews.
- Contribute and support development and maintenance of continuous quality improvement systems, policies and procedures.
- Maintain the privacy and confidentiality of every client in our services.
Essential requirements
- Qualifications in human services fields and demonstrated experience working with people at risk or complex needs.
- Demonstrated experience in case management practices and strategies to support vulnerable people.
- Demonstrated experience in completing client assessments and plan development.
- Demonstrated experience in developing relationships and working with people with complex needs.
- An understanding of trauma-informed practice principles
Requirements (or ability to obtain):
- A valid current Western Australian Driver’s Licence
- First Aid Certificate
- Working with Children Check
- National Police Clearance
- Form 395 Clearance from Department of Communities
