Moorditj Koort Aboriginal Corporation (MKAC) is an Aboriginal community controlled, not-for-profit organisation supporting the health and wellness of Aboriginal and Torres Strait Islander peoples living in Western Australia.
The Vision is to contribute to healthy and flourishing Aboriginal communities across WA by providing access to a holistic one-stop-shop for health and wellbeing support.
Position Summary:
This position holds significant importance within the Aboriginal Wellness Centre Program at Moorditj Koort, overseeing the coordination of Allied health and specialist clinics. Responsibilities include maintaining frequent communication to assist clients in attending appointments, managing follow-up referrals, and organizing pathology services in preparation for the clinics.
Duties and Responsibilities:
- Liaise with external specialist (Nephrology, Endocrinology, ENT) and stakeholder to schedule clinics.
- Coordinate client bookings working closely with Care Coordinators and Community Transport Drivers.
- Assis Clients internal/external specialist referrals and provide fast track access to health and complimentary services.
- Support clients to attend a culturally appropriate GP appointments to obtain specialist referrals and pathology for MKAC specialist clinics.
- Follow-up documentation and client communication post clinics e.g. delivery of dictation machine, re-calls, filing.
- Preparation of clinics, ensuring equipment is ready for the day and documentation as required for each clinic (Nephrology, Endocrinology, ENT & Allied health)
- Empowering clients to make informed health and wellbeing choices.
- Support health professionals and health services to develop culturally appropriate and safe policies and practices.
- Coordinates and maintains an effective patient record system with the booking and communication of patient services within Aboriginal Wellness center (AWC) including updating clients.
Skills, Abilities and Knowledge
- A high level of verbal and written communication skills;
- Demonstrated analytical and problem-solving skills;
- Demonstrated ability to show initiative and work autonomously.
- Proficiency with literacy of information and communication technology including desktop computers, word processing, spreadsheet and database software packages.
- Understanding of local Aboriginal Health issues.
- Demonstrated planning and organisational skills, especially in relation to setting priorities and meeting deadlines.
The successful candidate must have the following essential certifications.
- Cert IV in Primary Health Care Practice or in relevant field such as Nursing
- AHPRA Registration
- Valid WA Driver’s license
- Current Working with Children Check
- National Police Clearance
- COVID-19 Vaccination Certificate
- First Aid or willing to obtain.
