South West Healthcare (SWH) is seeking a highly skilled and experienced Registered Nurse to coordinate District Nursing care to people living in the Camperdown and surrounding areas with chronic and complex illnesses by preventing avoidable hospital admissions and reducing the frequency and duration of hospital admissions.
The positon will collaborate with services in the community – Acute, Aged Care and Specialist Care to reduce duplication and create effective health pathways for clients.
- This is a Permanent, Part Time position (64 hours per fortnight).
You will be registered with AHPRA as a Registered Nurse with relevant post graduate qualifications and a minimum of 5 years' experience. Demonstrated high level clinical knowledge and skills in assessment, planning and implementation of services and an ability to communicate effectively with all stakeholders.
