Where you'll be working
Assertive Outreach Team, Pennant Hills
The CNS is responsible for providing Complex Care Coordination for consumers with chronic mental health illness, and Assertive Outreach support over a rotating roster, along with providing evidence-based, strengths focused, recovery orientated assessment and treatment within a multi-disciplinary team and coordinating consumers’ recovery plans. The role is also responsible for non-clinical tasks including participation in quality improvement activities, clinical supervision and participation in administrative responsibilities as required.

Candidates will need to meet the following criteria:- Current registration as a Registered Nurse with the Australian Nurse Health Practitioner Regulation Authority (AHPRA) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification or such other qualifications or relevant equivalent experience.
- Demonstrated knowledge and ability to mentor and coach nursing staff to enhance professional development.
- Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
- Proven extensive clinical experience in community mental health services and evidence of effective planning, implementation and evaluation.
- Current NSW Drivers Licence.
