Community Nurse/General Practice Nurse

IMPACT Community Services' (IMPACT) GP-led Integrated Health Service offers a range of high-quality medical, community and allied health services under one roof. The team of dedicated healthcare professionals is committed to delivering comprehensive and holistic, patient-centred care.

We are currently recruiting a Community Nurse/General Practice Nurse to join our team. This is a casual role up to 76 hours per fortnight based at Agnes Water on the Discovery Coast.

About the role

• Assess, plan, implement, and evaluate individualised patient care.
• Refer clients onto appropriate health and community services and support groups.
• Maintain comprehensive client records and data collection.
• Liaise and foster collaboration with existing health and community care agencies.
• Provide an innovative approach to flexible service delivery that will encourage access and participation for all people in the community.
• Maintain relationships and liaise with key stakeholders.
• Undertake Practice Nurse duties – triage, treatment room, health care assessments, chronic disease management, wound management.
• Undertake Community Nurse duties – referral and navigation, in home services, wound management, palliative care, health education and promotion

To make a difference in this role, you will have:

• Registration as a Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA).
• Experience in a general practice or primary care setting.
• Mandatory annual competencies (First Aid/CPR, Manual Handling, Hand-washing)
• High level of professional interpersonal skills (customer service, communication skills, conflict resolution, organisational and time management)
• Ability to work independently and collaboratively within a multidisciplinary team.
• Sound administration skills.
