Based in Salisbury South, Port Pirie or Whyalla, as our invaluable Complex Support Coordinator, you will support older individuals with more complex aging concerns to maintain their independence and continue living in their own homes by prioritising client capabilities and choices.
As the Complex Support Coordinator, your primary responsibility will be to manage, refer, recommend and coordinate the delivery of services to clients experiencing more Complex Support needs, including managing any Client concerns that impact on their ability to stay in their home. By fulfilling this role effectively, you will contribute to the overall well-being and satisfaction of our clients as they receive the care and services, they require to maintain their independence.
You’ll also make an impact by:
- Conducting comprehensive assessments of clients needs, including physical, emotional, cultural, and social aspects
- Working with the client’s current Home Care Coordinator to support learning of risk identification and mitigation strategies
- Attending home visits with allied health/nursing services to conduct extensive reviews in addition to current schedules of assessments
- Liaising and working with other Helping Hand client service areas to ensure that the overall delivery of services to clients is promoted and delivered as a seamless service
What you’ll bring
To succeed in this role, you will have:
- A degree qualification in a related health discipline including Nursing (Registered/Enrolled Nurse), Allied Health, Mental Health or Disability Support
- Extensive experience supporting vulnerable and/or complex client support needs
- A strong theoretical approach to working with ageing population and demonstrated practical application of this approach
- Ability to demonstrate an understanding of community resources and supports
- Ability to maintain data collection and outcome measurement systems
- Excellent organisational skills with ability to prioritise and complete work within established deadlines
- Excellent interpersonal and highly developed written communications skills
- Travel between Metropolitan Adelaide and Country SA locations is required.
- Valid NDIS Worker Screening clearance or willing to obtain one
- Department for Communities and Social Inclusion (DCSI Clearance)
- Own car or access to vehicle and current Drivers Licence
