About the role:
An opportunity has arisen for Support Workers to provide exceptional 1:1 support as identified in Individual Personal Centered Plan, Positive Behavior Support Plan, Functional Assessment as well as day-to-day identified activities and tasks. This particular role involves working in a team-based, home-based or community setting on a roster basis with the ability for overnight, weekends requiring the ability to use good judgment with an emphasis on active support being essential.

Duties and Responsibilities include:
- Follow daily schedules including, routines, identifying client needs, cleanliness, activities and shift guidelines
- Follow set rosters and time frames and be prepared to do some weekends and sleepovers.
- Provide quality active support to clients including completion documentation.
- Medication assistance and management where required.
- Safety, well-being and support of choice and independence of clients.
- Confidentiality and privacy of clients.
- Work with clients to achieve the goals identified in their support plans.
- Be a positive and constructive team member who role models professional behaviour at all times.
- Set and maintain high standards of operation in accordance with the policies procedures and program guidelines.
- Contribute to effective risk management and manage all incidents, hazards, and complaints in accordance with company policy and legislation.
- Contribute effectively to the promotion of equal opportunity and non-discrimination in the workplace.
- Provide clients with a physically and emotionally secure environment and promote their physical, emotional, social, and intellectual wellbeing.
Desired:
- Familiarity with the Community Services Sector, in particular NDIS frameworks.
- Cert III in Disability Support, Youth Work or relevant experience.
- Demonstrated capacity to work independently as well as part of a team.
- Ability to deal appropriately with stress, maintain composure and apply effective engagement strategies with clients.
- Capacity for self-motivation, ability to cope with changing work functions, workloads, and tasks.
Required:
- Competent IT skills.
- Current C Class Drivers’ Licence and reliable vehicle
- A current Blue Card and NDIS Workers Screening or ability to obtain
- A current Police Check or ability to obtain one.
- Current First Aid and CPR Certificate.
- NDIS Orientation, Effective Communication and Safe & Enjoyable meals modules.
