How you’ll play your part to support individuals maintain choice and control in their homes and community
You will be required to provide in-home and/or community support to people living with disabilities.
This may be through personal care, domestic assistance, community access, meal preparation etc. Alongside this, having the responsibility for monitoring a client’s progress, reporting any concerns and maintaining accurate case notes.
Qualifications & experience
- Six months of experience within a similar role or hold a qualification in Individual Support, Aged Care, Disability or Community Care
- NDIS Worker Check (NDISWC)
- NDIS Worker's Screening Module - Certificate of Completion
- COVID Infection Control Module - Certificate of Completion
- Current First Aid Certificate & CPR
- Proof of Vaccination
- 100 Points of ID and right to work in Australia
- Driver's License and reliable insured vehicle
Tasks & responsibilities
Providing care and support to people living with disability in line with their personalised goals and care-plans, including but not limited to:
- Personal care and hygiene tasks (showering/dressing/grooming)
- Domestic assistance / housekeeping
- Meal preparation
- Assist with medication / prompting
- Community access
- Transport (shopping, attending appointments)
