Position Title:End of Life Care Coordinator

Employment Type: Permanent, Full-Time

Hours per week: 38 hours

Position Grade: Multi-grade position: Clinical Nurse Consultant, Grade 2 / Social Worker Level 4

Salary: $114,042.00 - $125,557.29 base salary per annum

About the role: This role will coordinate End-of-life Care (EoLC) processes and procedures in the last year of life including palliative care, goals of care, advance care planning and our response to Voluntary Assisted Dying (VAD), across SVHNS, including education and training.

This role will also ensure ongoing alignment with polices, standards and legislation regarding EoLC for SVHNS, and engages in monitoring, identification and reporting on risks associated with EoLC..

About you:

We are seeking a skilled and energetic nurse or social worker who possess a passion for person centred care.

Selection Criteria:

  • Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries
  • Currently registered as a Registered Nurse Division 1 with the Australian Health Professionals Regulation Agency (APHRA) OR degree in social work from an accredited course providing eligibility for the membership of the Australian Association of Social Workers (AASW)
  • For nursing, at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field. In addition the employee must have approved postgraduate nursing qualifications relevant to the field in which he/she/they are appointed or such other qualifications or experience deemed appropriate by St Vincent’s Hospital, Sydney. For social workers, experience in oncology, palliative care, end-of-life care, advance care planning or in caring for patients with complex and/or life limiting conditions
  • Demonstrated ability to work autonomously and with a high degree of motivation and capability.
  • Demonstrated ability to work effectively, engage and collaborate with key stakeholders and foster teamwork and collaboration.
  • Experience in implementing and monitoring compliance with regards to policy and procedure
  • High level interpersonal negotiation and communication (written and verbal) skills
  • Demonstrated understanding of clinical governance health care systems including quality improvement, monitoring and analysing data.
