We are seeking applications from Enrolled Nurse / Registered Nurse or candidates that hold a Allied Health discipline or related exeperince in Demetia care to join our team as an Engagement Facilitator - Treehouse4Two Retreat Program.  

The Treehouse4Two (TH42) Retreat Program is a grant funded program that will improve the lives of people living with Dementia and their carers by increasing early access to Respite and support services. 

Treehouse4Two Retreats provides:

  • Access to a person-centred program for both carers and people living with dementia across the Macedon Ranges and Hepburn Shires of regional Victorian.
  • A wellbeing and education focus in a peer learning environment to support participants to live well at home for as long as possible; improve their quality of life, reduce the physical and psychosocial burden of caring; and delay early entry into permanent aged care.
  • The opportunity to build carer knowledge, skills and capability in their role as a carer for someone living with dementia.
  • Increased awareness of the benefits of respite for people living with dementia and their carers
  • Support for the person living with dementia to continue to live well.

Program Overview

  • Consists of both structured and unstructured activities for both carer education and people diagnosed with dementia and adapts along with group dynamics and individual needs.
  • Operates over a 3 day / 2-night period and is held monthly with approximately 6 couples (the carer participates in an Education program and the person diagnosed with dementia participates in a wellbeing program). All participants come together for meals and couples have their own accommodation on site.

Role Statement

The Engagement Facilitator is pivotal to ensure the individual needs of participants living with dementia are met in the context of a small group setting while the carers are in education sessions.

This role delivers a program of wellbeing and learning sessions including topics about dementia, ways to maintain physical, emotional and psychological wellbeing, grief and loss, self-care and other resources/supports available; and encourages peer learning, the sharing of stories and experiences through open and supportive discussions and reminiscence.

The role links with the Carer Education Facilitator to ensure continuity of support approaches for the person living with dementia and is a role model for carers of appropriate communication and behaviour.

Details of Appointment

Fixed-Term / Permanent position (03/06/2024 - 06/07/2025) working 32 hours per fortnight

Salary:  Dependent on experience, as per EBA + 11% superannuation + access to salary packaging

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30th November 2019.  Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
