Join Our Client's mission to provide exceptional care for Senior Australians! As an Intake Assessment Officer, you'll play a crucial role in screening, triaging, and assessing client referrals.
About the Role:
- Screen and assess CHSP referrals via My Aged Care portal
- Collaborate with HCP Case Manager for Home Care Package approvals
- Conduct telephone assessments to determine health and social needs
- Liaise with internal and external stakeholders for a responsive intake experience
- Experience in assessing and managing clients with complex health and social needs
- Understanding of issues affecting seniors, including special needs groups
- Excellent communication, negotiation, and analytical skills
- Proficient in Microsoft software packages
- Current driver's license and satisfactory police check
- Professional interaction with clients, families, and external stakeholders
- Reporting to the Home Care Service Manager
- Ensuring a responsive intake experience for clients and promoting the organization's services
- Working to a fast and efficient pace to ensure all referrals are attended to in a quick manner.
