Position Overview:
Healthcore is seeking an enthusiastic Client Intake Officer with NDIS experience to join our Scheduling Team. In this crucial role, you will be the first point of contact for referrers and patients, connecting them with our team of allied health professionals.
Key Responsibilities:
- Collect and record all relevant information from referrals, including diagnoses, risks, barriers, and issues.
- Triage, analyse, and coordinate referrals.
- Utilise a client-centred approach to ensure efficient and timely access to our services.
- Maintain accurate records and information within our CRM.
- Schedule appointments and assist with workload management for team members.
- Create and facilitate the delivery, completion, and submission of NDIS Service Agreements.
- Work closely with allied health managers to direct service requests to the most appropriate professionals within our team.
- Develop, foster, and maintain relationships with our referrers.
About You:
Experience in positions such as NDIS support coordination, therapy coordination, social work, or other allied health-related roles would be highly desirable. This background ensures you possess a thorough understanding of working with participants and their families, as well as a comprehensive knowledge of NDIS guidelines and their application in the provision of allied health services.
