The Victorian Government’s Department of Health and Human Services (DHHS) is working in partnership with the Brotherhood of St Laurence (BSL) and Meli to develop and test a new practice framework, resources and tools for young people transitioning from out-of-home care into independence. This role will apply a key worker approach to Leaving Care eligible clients, families and services and agencies connected to the young people.
The key objectives of this role are to:
- Provide case management and support to referred Regional Leaving Care clients, who have multiple or complex needs and/or issues and require support from a range of services and sectors.
- To work with the individual, family and children and collaborate with them to develop a Client Support Plan, holding the complete picture of overall needs, strengths, goals and priorities, to achieve improved client outcomes and self-management.
- Network and collaborate with a broad range of services to ensure an integrated service response to young people in the program.
- Be the primary contact for the individual and collaborate with them to develop a Client Support Plan, holding the complete picture of overall needs, strengths, goals and priorities, to achieve improved client outcomes and self-management.
- Full-time role, working 38 hours per week.

- Relevant tertiary qualification in Social Work, Welfare/Youth Work or associated discipline or relevant experience working in this space.
- A sound understanding of issues confronting young people leaving the statutory care environment.
- Proven ability to engage and case-manage young people at risk and young people exhibiting challenging behaviours.
- An understanding of the impact of trauma on young people and the ability to work from a trauma informed approach, to effectively and constructively work alongside the young people, including statutory clients and those with challenging behaviours
- Understanding of the purpose and expectations of the Victorian Child Safe Standards and demonstrated commitment to contributing to a child safe organisation in both practice and culture.
- Current Victorian Drivers Licence.
