Focus Care is looking for an experienced Home Care Package Care Manager looking for the next step in Community Aged Care.
The Manager Community Aged Care will provide leadership to a team of Community Aged Care Coordinators and will have extensive knowledge of the Home Care Packages Program and the reform process. This role will work with the Executive team to ensure a smooth transition to Support At Home for the organisation and the community we serve.
Key responsibilities of the role:
- Oversee the Home Care Package Program and ensure best practice Community Aged Care so that our home care services meet and exceed the Aged Care Quality Standards
- Lead, mentor and supervise a team of Community Aged Care Coordinators, monitoring performance and providing direction to ensure best practice Care Management.
- Applying extensive experience as a HCP Care Manager as well as excellent mediation skills, support the team by managing complex client issues and conditions such as Mental Health, Hoarding and Squalor, Abuse etc.
- Ensure best practice care planning, assessment and review processes and develop and implement training to enable a depth of geriatric literacy across the Community Aged Care Team.
- Monitor legislation, standards and program guidelines, and the reform process to ensure knowledge is current and the organisation is compliant and, in a position to plan for reform.
- Work with the Executive team to plan and work towards the transition to Support At Home.
- Work with senior management to develop and implement a plan for continuous improvement that enables Focus Care’s strategic plan to be achieved.
About you:
- An experienced HCP Care/Case Manager, with knowledge and understanding of the Aged Care Quality Standards and ideally experience in participating in a Quality Review.
- Demonstrated experience in managing teams, in particular, the mentoring, supervision and coaching of staff.
- Excellent verbal and written communication skills with experience mediating difficult situations.
- Enthused by the reform process, and the opportunities it presents.
- High level problem solving and critical thinking skills.
- Tertiary qualification, ideally Health related
