Now is your chance to shape the future of Mental Health and wellbeing in Victoria to ensure that everyone is supported and includedMental Health and Wellbeing Locals are being established across Victoria focusing on care and support for people aged 26 and over who are experiencing Mental Health concerns – including people with co-occurring alcohol and drug treatment and care needs and their family, carers, and supporters. Eastern Health has partnered with Wellways, Inspiro and Oonah to deliver the Yarra Ranges Local which will offer a "how can we help?" and no wrong door approach making it easier for people to access the support they need close to their home, improving capacity of individuals to engage in our community and respond to any future psychological distress.About the roleWe are seeking an experienced Mental Health clinician to join the Yarra Ranges Local in a new position as Grade Two Social Worker. You will play a vital role in ensuring that consumers, carers and their families receive valuable and supportive Mental Health care. This role will ensure decisions are always either directly or indirectly aimed at ensuring the person with lived experience is at the centre of all we do and that the ethos and values of clinical professionalism are imbued within the culture of the organisation and play an integral role in changing and shaping the way Mental Health and wellbeing services and supports are delivered to the Yarra Ranges community.The Grade Two Social Worker will provide services to people experiencing a range of Mental Health and wellbeing challenges. Services will be provided within a strength based, person centred, trauma informed, goal and recovery-oriented framework. The Social Worker will work alongside the individual consumers and their carers with an individual plan designed to promote, foster and sustain personal recovery.About youTo be successful in this role you will possess:

• A degree qualification in Social Work that has been approved by the Australian Association of Social Workers (AASW) for membership as a Social Worker.
• A strong commitment to high quality, person centred care, that respects and upholds the human rights of individuals
• Demonstrated experience integrating lived/living experience expertise throughout various models of care
• Expertise in delivering counselling and therapeutic interventions that are grounded in evidenced based practices in line with the unique needs, preferences and circumstances of each participant.
• Demonstrated experience in providing family/carer inclusive practices and ability to work with family and carer supports available within the Local and externally
