About the Role:
As an integral part of our commitment to excellence in support services within Enabling Pathways, your role as an NDIS Coordinator will revolve around upholding the highest standards for our NDIS Programs in the Community, Supported Independent Living (SIL), and Specialist Disability Accommodation (SDA). Your responsibilities include the cultivation and maintenance of relationships, coupled with the facilitation of comprehensive training programs for support workers. By actively contributing to these initiatives, you play a crucial role in ensuring that our participants receive tailored support that precisely meets their individual needs and requirements.
As a NDIS Coordinator, your role will involve:
- Develop individualised care plans for CA/SIL/SDA participants.
- Oversee the practical application of care plans for specified support.
- Monitor and adjust care plans for optimal participant outcomes.
- Support planning and logistics for participant transitions.
- Liaise with participants, decision-makers, and support coordinators for service agreements.
- Guide participants through intake processes and assist with documentation.
- Maintain Medication Health Passports for participants, ensuring accurate records.
- Assess and update medication management protocols for participant safety.
- Act as a central point of contact for participants, support workers, and healthcare providers.
- Provide guidance on navigating the NDIS and accessing resources.
- Collaborate on financial management for participants.
- Advocate for participants' rights and address concerns.
- Conduct regular visits to residences and participants.
- Ensure continuity of support, being accessible for emergencies outside regular hours.
- Identify training needs for assigned programs.
- Complete Risk Assessments for each participant regularly and submit as required.
- Create and manage CA/SIL/SDA rosters and care schedules, adhering to award guidelines and NDIS pricing arrangements.
- Approve timesheets and implement strategies to minimise overtime.
Skills & experience:
- In-depth understanding of the NDIS framework, principles, and guidelines.
- Knowledge of disability rights, legislation, and best practices in disability support.
- Experience in care coordination or a related field, particularly in disability support or healthcare.
- Demonstrated experience in conducting assessments, creating care plans, and monitoring participant progress.
- Familiarity with person-centred approaches and individualised support planning.
- Proficiency in using relevant software and technology platforms for documentation and communication.
- Ability to work independently and as part of a team
- High level of PC literacy and Proficient with Microsoft suites
- Exceptional organisational skills and adept at prioritising your workload
- Highly developed interpersonal skills
- Excellent written and verbal communication skills

Mandatory Requirements
- Diploma in Individual Support or equivalent.
- Proof of citizenship (passport, birth certificate or citizenship certificate) or VISA if applicable.
- Current QLD Open Driver's License.
- Must hold a Qld Blue Card
- NDIS Workers Screening Yellow Card.
- Hold a current First Aid (HLTAID011) and CPR (HLTAID009) Certificate.
- A clean National Police check less than 6 months old.
- Completed NDIS Orientation Module.
- Willing to undergo training in specific courses to support the participant’s individual needs
- Ability to communicate effectively Excellent communication (written and verbal) and interpersonal skills, including working and communicating effectively with clients, their families, community agencies and other professionals.
- Ability to work in a busy environment, both independently and as a part of a team.
- Comply with & understand all privacy and confidentiality policies.
