The Support Coordinator will be responsible for assisting NDIS participants with complex needs to build personal capacity to make decisions and choices about the implementation of all supports in their NDIS plan, including mainstream, informal, community and funded supports. The role also focuses on addressing barriers and reducing complexity in the support environment whilst also assisting the participant to connect with NDIS and other supports, negotiate solutions with multiple stakeholders and build capacity and resilience.
Your key responsibilities will include:
- Provide exceptional Support Coordination to our NDIS Participants
- Assist to identify goals, needs and opportunities to build capacity to participants
- Support participants to exercise choice and control
- Link participants with appropriate Service Providers as well as community and main-stream services
- Liaise with stakeholders to promote client safety, and to identify and overcome barriers to client social and emotional well-being.
- Maintain case notes and produce reports and other written material as required.
- Prepare plan review reports and other requested reports.

- Excellent written and verbal communication skills
- Ability to adapt communication style to meet people’s needs
- Strong administrative skills; time management, coordination of tasks, efficient work practices
- Excellent computer literacy
- Ability to work with minimal supervision
- Strong working knowledge of NDIS
- Experience working in fee for service or billable hours environments and ability to prepare and work within clients budgets,
- Demonstrate your ability to work autonomously and within a team environment.

Mandatory requirements:
- NDIS Worker Screening Check (or willing to obtain)
- National Criminal History Record Check (Police Clearance)
- You must hold a current driver's licence
- DHS Working with Children check
- Previous experience in providing Coordination of Supports to NDIS Participants
Remuneration:
- Position is classified Level 4.1 under the SCHCADS award;
- Salary $37.54 an hour
