Senior Ndis Support Coordinator

Nunyara is an NDIS Registered service who is committed to improving the health and delivery of care services to Aboriginal people in the Whyalla region. Our aim is to ensure our community have access to culturally appropriate Community Connection Programs, NDIS Support Coordination and meaningful Case Management services and as such, we are expanding our Integrated Care Team to build our capacity to provide a range of services to Aboriginal people within the Whyalla region with a disability. 

The successful applicant will be responsible for providing support coordination services to NDIS Participants who need services from multiple providers. The Senior Support Coordinator will assist participants to receive the most out of their NDIS plan and works in partnership with participants by regularly reviewing progress of individual plans and assisting participants in achieving set goals.

Applications are invited from highly motivated people who can demonstrate their previous experience in NDIS Support Coordination.

The Senior Support Coordinator is responsible for a range of tasks including:

Organisational Responsibilities

1. Participate in performance appraisals, Nunyara training, group supervision, team meetings and professional development activities.
2. Maintain appropriate personal and professional boundaries.
3. Working as part of the team based on an ethos of collaboration, co-operation, respect, and mutual support.
4. Other duties as directed by the Manager.

Working collaboratively with existing NDIA service providers and stakeholders such as: 

1. Aboriginal Disability Liaison officers
2. Health Liaison Officers
3. Justice Liaison Officers
4. Partners in the Community (Mission Australia) 
5. NDIS planners / complex planners

Support Coordination Responsibilities

1. Connect participants to NDIS and other supports.
2. In partnership with participants, develop and regularly review progress of individual plans and support strategies.
3. Regular contact with each participant. Feedback from participants.
4. Monitor plan budgets and support effectiveness.
5. Build capacity and capability to understand their plan, navigate the NDIS and make their own decisions.
6. Work with other people in the participant’s circle, such as their friends, family, therapists, and doctors, to meet the goals.
7. Monitor the progress of plan goals and help participants in achieving them.
8. Assist in preparing for initial and ongoing annual planning meetings with the NDIS.
9. Assess and complete risk assessments for each participant. Risk assessments are updated and reviewed at least on one a year and as needed. Concerns of risk are noted with the Manager.
10. Regularly review and refine support strategies to minimise risk and optimise best outcomes for participants.
11. Build and maintain working relationships with disability and mental health services and other government and community agencies.
12. Manage time effectively between participants and document the coordination that has been provided in the participant management system.
13. Complete compulsory NDIS progress reports which outline progress towards NDIS goals, linkages to services, fund utilisation and recommendations for future.
