- Permanent, full-time position available from April 2024
- Salary range: Allied Health Professionals – community Development Worker Class 2B Years 1-3 dependent on experience, as well as all applicable allowances and penalty rates.
- Annual leave paid as per EA with monthly paid day off for full time staff.

Mansfield District Hospital (MDH) is located in Victoria’s scenic high country, at the foot of Mount Buller and on the shores of Lake Eildon, only two hours from Melbourne. We have created an exceptional health service for the Mansfield and surrounding community life stages and critical health needs, including acute hospital care, urgent care, operating theatre, obstetrics with a midwifery group practice model, aged care, regional district nursing service for in-home care and our allied health services.
We are seeking a Care Coordinator for the Mansfield Support at Home service (MSH) who is responsible for overseeing the operations of the Mansfield Support at Home Service, ensuring the efficient and effective provision of safe and high-quality health care to people in our community and at MDH. The role will manage the Home and Community Care (HACC) Program for Younger People (PYP)and Commonwealth Home Support Program (CHSP). The role is also client centred, providing support for two cohorts of clients. Firstly, younger people under the age of 65 years with disabilities whose capacity for independent living is at risk (HACC PYP). Secondly those clients over the age of 65 years (50 years and over for Aboriginal and Torres Strait Islander people) who receive services under CHSP, to assist them to continue to remain living at Home and in their community. The role contains both clinical and management responsibilities.
To be successful in this role you will possess clinical care experience, strong interpersonal and communication skills, an ability to relate to staff, patients, relatives/ carers and other health care providers. You will have a passion for community care, leadership experience, a willingness to learn and be committed to modelling the Great Care at MDH values and expectations.
