About the role

  • The purpose of this role is to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care for First Nations consumers/persons with complex clinical needs within Outpatients at RBWH and for the provision of specialist nursing advice, consultation, leadership and education to inter-professional team members as relevant to the specific context of the role.
  • The role will:
  • Coordinate consumer/person centred care across the consumer/person journey.
  • Create partnerships, for example, providing a central point of communication, engagement and coordination for all stakeholders involved in the consumers/persons care.
  • Improve consumer/person outcomes through the delivery of evidence-based care.
  • Facilitate systems improvement - providing a leadership role within the organisation and act as an agent of change.

About you
