Form of workKidney Outpatient Department are looking for Nurse Navigator – Kidney to join their team. The Nurse Navigator Service is a state-wide model of care designed to provide holistic support to patients with complex health conditions requiring an expert level of healthcare coordination. This integrated and proactive approach to health care service delivery, helps ensure that patients receive the most appropriate service when and where they need it.

About the Role
- Demonstrate advanced application of clinical and professional leadership skill and specialist knowledge to achieve integrated patient care and reduce fragmentation in the patient journey.
- Apply advanced knowledge and skill to engage patients, family and caregivers, encouraging self-management and health literacy supporting informed decision making.
- Exercise advanced clinical judgement, informed by an evidence-based model using critical thinking and ethical decision-making processes, in the provision of patient-centred care coordination across the patient journey.
