The Nurse Navigator Service is a state-wide model of care designed to provide holistic support to patients with complex health conditions requiring an expert level of healthcare coordination. This integrated and proactive approach to health care service delivery, helps ensure that patients receive the most appropriate service when and where they need it.

• Demonstrate advanced application of clinical and professional leadership skill and specialist knowledge to achieve integrated patient care and reduce fragmentation in the patient journey.
