Job description

Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
About the role
* The purpose of this role is to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care for First Nations consumers/persons with complex clinical needs within Outpatients at RBWH and for the provision of specialist nursing advice, consultation, leadership and education to inter-professional team members as relevant to the specific context of the role.
* The role will:
* Coordinate consumer/person centred care across the consumer/person journey.
* Create partnerships, for example, providing a central point of communication, engagement and coordination for all stakeholders involved in the consumers/persons care.
* Improve consumer/person outcomes through the delivery of evidence-based care.
* Facilitate systems improvement - providing a leadership role within the organisation and act as an agent of change.
About you
