About the role
The Health on the Streets (HoTS) Program is an innovative program that delivers a mobile outreach and primary care services to vulnerable people including people experiencing homelessness and those sleeping rough on The Central Coast. HoTS provides health assessments and primary care treatment by an Outreach Nurse supported by the HoTS Peer Worker.
The Outreach Nurse - HoTS will work collaboratively in a team with the Assertive Outreach Workers to build rapport and connect with people experiencing homelessness in the community at regular community hubs and other locations. We have two (2) part-time roles available, one working two (2) days per week and the other role working three (3) days per week.
The Outreach Nurse – HoTS will be required to undertake a range of tasks including but not limited to the following:
- Work with the HoTS team including management, team leader and peer worker to establish program structure, activities, and resources.
- Conduct health and mental health assessments and provide immediate basic primary care treatment when necessary.
- Make appropriate health and community referrals as needed.
- Maintain an inventory and supply of clinical equipment and stock.
- Promote and provide education about the program and homelessness to health professionals and community including attendance at external meetings and events.
Key Selection Criteria
- Relevant tertiary qualification; Registered Nurse with current AHPRA registration
- Accredited nurse immuniser certificate
- Previous experience in alcohol and other drugs and/or mental health
- Previous experience in a similar clinical position
- Sound understanding of mental illness, comorbidity, and the complexities of homelessness with an ability to independently conduct clinical assessments.
- Proficient with venepuncture
- Demonstrated knowledge of current approaches to trauma-informed and person-centred practice
- Demonstrated effective clinical communication using information technology and sound written, verbal and interpersonal skills.
- An understanding of, and a willingness and ability to participate in the physical demands of the role. These may include working outdoors in changeable environmental conditions, working in areas inhabited by homeless persons, and dealing with challenging individuals.
- A valid Working with Children Check (WWCC) and a Criminal Record Check (CRC) issued within the last 12 months that includes working with vulnerable groups
