What you'll be doing

The primary responsibilities of the Discharge Planner (RN) are: Co-ordination and facilitation of post-discharge care for complex patients; Early identification of patients requiring follow-up, outreach services and community care; Act as a resource to patients and their carers, hospital, outreach and Community health staff in relation to discharge planning practice; work closely with the patient journey facilitator and multidisciplinary team.

Selection Criteria:

1. Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse (Division 1).
2. Extensive clinical experience in a broad range of clinical settings with extensive assessment, time management and care planning skills.
3. High level interpersonal and negotiation skills across a multidisciplinary team.
4. Demonstrated high level verbal and written communication skills.
5. Demonstrates an understanding of the Continuous Quality Improvement concepts.
6. Computer literacy and understanding of relevant health related information systems.
7. Demonstrated understanding of person centred care and NSW Health Core Values.
