About the Role
As a Registered Nurse (RN) – Healthcare Coordinator Information you will be responsible for coordination of clinical care to eligible patients. You will liaise with members of the multidisciplinary Healthcare Coordination Forum (HCCF) team regarding appropriate patient care for cases where a high level of coordination and/or situational awareness is required, or where there are concerns about the member’s level of engagement with relevant stakeholders.
To be successful in this role, you will have:
- The highest levels of personal integrity and ethical behaviour.
- Current unconditional and unrestricted registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Minimum of 2 years full-time Post Graduate experience within the previous 5 years in an acute health care, primary health care, or military setting (or part-time equivalent).
- Recent participation in Continuous Professional Development (CPD) activities in accordance with AHPRA.
- Current Cardiopulmonary Resuscitation (HLTAID009) certification.
About Campbell Health Centre
Campbell Health Centre is located in the coastal Perth suburb of Swanbourne at Campbell Barracks. The Health Centre provides general medical and daytime inpatient services, as well as dentistry, physiotherapy psychology, and pharmacy. Its standard operating hours are 0730-1530, Monday-Friday.
