About the Role
As a Senior Social Worker, you will deliver specialised social work services within a multidisciplinary team based in a holistic service that specialises in Aboriginal and Torres Strait Islander Primary Health Care. This role has a specific focus on supporting clients at risk of developing Chronic disease.
- Deliver advanced level Social Work clinical services on an individual and/or group basis to clients within the Inala Indigenous Health Service targeting chronic disease prevention, treatment and management associated with Cardiovascular Disease, Type 2 Diabetes and Chronic Respiratory Disease.
- Undertake high level independent Social Work interventions with clients with complex needs within the Inala Indigenous Health Service with infrequent need for direct clinical practice supervision.
- Provide professional social work leadership, knowledge, and skills in the development of clinical practices, procedures and protocols that support the continuum of care engaging cross-discipline referencing where relevant.
- Provide high level clinical advice to professional and operational supervisors, relevant service managers and other stakeholders regarding service delivery.

Your current CV or resume, including two referees.
