Service Coordinator - In Home Community Support 

  • Eltham location hybrid/ flexible working arrangements
  • Competitive salary including salary packaging options
  • 6 Month contract Full time role with an opportunity for ongoing

The Opportunity

We are looking for a Service Coordinator to join our In Home Community Support team in a Full Time capacity on a 6 month contract with a potential for ongoing work.

Responsibilities include but are not limited to:

  • Coordinate and schedule services between casual staff and clients
  • Ensure all actions and liaisons are documented in the appropriate systems.
  • Act as a liaison between clients and the support worker team, managing support services in the client’s home and community.
  • Maintain comprehensive records of all communications and actions in the client management systems.
  • Provide a high level of customer service and administrative support to the In Home Community Support clients and team.
  • Ensure timely follow-up of referrals and service requests.
  • Prioritize urgent client service requests based on risk assessment and understanding of the client’s needs and goals.
  • Capture essential data for service planning and reporting.
  • Ensure all interactions comply with national privacy and confidentiality requirements.
  • Monitor email inboxes and website portals for new service requests.

Skills & Experience:

  • Recent experience in customer service, rostering, and administration within the health, aged care, or disability work sector.
  • Good understanding and knowledge of a broad range of social and health issues to assist clients with service access.
  • Demonstrated ability to work flexibly and contribute to quality and continual improvement.
  • Ability to contribute to workplace change in a positive manner.
  • High level of phone, communication, and interpersonal skills.
  • Understanding of and experience in dealing with complex needs, behaviors, and sensitivities of people from culturally and generally diverse communities and backgrounds.
  • Demonstrated competency with appropriate client management systems and database applications.
  • Willingness to undertake training to gain a high level of competency in all appropriate client management systems.
  • Proven ability to work independently and as a member of a team.
  • Demonstrated high-level time management skills with a proven ability to prioritize and meet deadlines.
  • Proven high level of administration skills which enable you to effectively support with administration tasks as required.
  • Demonstrated proficiency in MS Office applications (Word and Excel).
