Australian Health Professionals (AHP) Disability & Home Care is looking for a Social Worker who are passionate about empowering people with disabilities to live their best life.
About this Opportunity...
In this community based role, you will be assisting with personalised evaluations and recommendations in practical tasks and daily activities working with NDIS participants, adults and children.
You will collaborate with those living with disability, their families and other allied health teams to ensure they receive a holistic support plan that meets their specific needs.
This role will suit someone who is already working and has the capacity to see a couple more clients.
About You...
This role requires you to be:
- Registered and qualified as Social Worker
- Have your ABN
- Committed to providing holistic person centred care
- Hold current National Police Check, Working with Children's check and NDIS Worker Check
- Driver's licence and access to own vehicle
- Experience working with NDIS participants
