Preference for a degree in social work, psychology, nursing, or other allied health professions, and eligible for membership to a professional association (or professional registration).
Core Responsibilities/Duties
Assist participants, carers and relevant supports to understand the NDIS; supporting participant full choice and control and independence.
Assist participants and their carers to identify needs and goals either over the phone or face to face. Develop clear action plans with key stakeholders. Attend face-to-face meetings in the office, community, or the home setting. Lead collaborative case conferences and distribute minutes to key stakeholders.
Identify gaps in services and research and refine support options in collaboration with participants/representatives. Research and implement new services to assist participants to meet their NDIS goals. Improve access to informal, mainstream, community and funded services.
Educate and build capacity of participants/representatives in their decision making.
Identify risks and barriers and demonstrate high level problem solving, analysis and decision making.
Communicate and build rapport and confidence with participants, carers, family, community, government and non government personnel.
Implement policy, procedures and work instructions in compliance with best practice and the NDIS code of conduct and practice standards.
Work with children and families at risk and develop streamlined service responses across government and non government sectors.
Manage time scale of individual plans, including tracking, follow up, and reviewing when required.
Excellent skills in report writing.
