Support Advisor/Registered Nurse

Buckland Homecare is a new and growing provider of Home Care Packages to the residents of the Blue Mountains region of Sydney. 

We are looking for a Support Advisor/Registered Nurse to look after a caseload of Level 3 and Level 4 clients which will average around 50. This position will be offered on either a part-time or full-time basis. 

About the role:

• Delivering quality care to HCP clients by enhancing their quality of life
• Act as the key contact person and advocate for clients in facilitating their Care Plan
• In partnership with the client, develop a goal-focused and consumer-directed Care Plan
• Ensure client needs and preferences are provided within their allocated budget
• Conduct Comprehensive Clinical Assessments including PAS or RUDAS assessments
• Liaise with referring agencies, ACAT, local health providers, and community services.
• Providing supervision and direction to care workers
• Perform any other reasonable duties as directed by Home Care Package Manager.

We are looking for someone with: 

• 1-2 years’ experience in Home Care Packages/Homecare
• Nurse Registration endorsed by AHPRA
• Have a minimum of 2 years’ experience as a Registered Nurse – Community Nursing an advantage
• Have a good understanding of contemporary aged care and clinical practice 
• Excellent written documentation and assessment skills
• Strong communication skills and the ability to problem solve and effectively resolve issues in a timely manner
• Have relevant knowledge and expertise in wound management and infection control
• Have an understanding of Work Health and Safety principles
• Be someone that strives for excellence, and high levels of achievement
• Valid NDIS Check, Current Drivers Licence, registered and insured motor vehicle.
• Sound understanding of Aged Care environment including Home Care Packages and Aged Care quality standards
