Our team comprises of over 30 staff counsellors, psychologists, employment coaches, support workers, and cleaners.
Role Description
This is a permanent part time role for a Support Coordinator, 30 hours per week growing into full time within 3 months. The Support Coordinator will be responsible for managing, monitoring, and maintaining care coordination for NDIS participants. This role will be in our office at Ormond with flexibility for some WFH.
Duties & responsibilities:
- Working with participants to achieve their goals in line with their NDIS plan.
- Providing ongoing Support Coordinator services to participants in line with their NDIS plan
- Utilise a person-centred approach to build capacity, choice, and control for the participants.
- Develop and implement support plans with clients and monitor their progress and outcomes.
- Identify and provide a wide range of solutions to complex challenges, barriers and risks that participants face.
- Undertaking work in line with the organisational policies, procedures, and values
- Complying with all relevant legislation and codes of conduct.
- Maintaining an up-to-date knowledge of local services and organisations participants can connect with
Experience & qualifications:
- Minimum of 1 year experience in NDIS support coordination
- Fluent in English
- Qualifications in community services, disability, or other health related disciplines, minimum of Cert 4
- In-depth knowledge of the NDIS rules and operational guidelines
- NDIS worker screen clearance & blue card
- First Aid and CPR certifications
- Excellent interpersonal skills to foster productive working relationships with an array of stakeholders.
- Permanent Australian working rights & current residence
- Computer literacy and proficiency in Microsoft suite
- Ability to work autonomously and take initiative.
