Brighter Futures Consulting provides Support Coordination, Specialist Support Coordination, Specialist Behaviour Supports, Music Therapy and Psychology services to Territorians living with disability or mental health diagnoses. We are looking to expand into other therapy services in the future.

We are currently seeking a pro-active Support Coordinator to join our team.
About the position:
The role of a Support Coordinator is to coordinate the supports for NDIS Clients and assist them in achieving their goals outlined in their NDIS funded plan. A Support Coordinators role is to proactively identify, develop, coordinate and facilitate access to supports and services alongside their allocated NDIS Clients.
Key responsibilities:
- Work with all stake holders including participants, families and carers to identify and access opportunities for skills development, education and employment
- Support and encourage clients to achieve their goals and provide support coordination services in line with their NDIS plans
- Develop networks and collaborate with stakeholders to proactively identify, plan, implement and evaluate supports.
- Prepare and maintain up to date and accurate case management notes and reports using clear and concise language on client progress.
- Liaise with a wide range of stakeholders, community services, government agencies, customers and service providers.
- Attend and contribute to team meetings, discussions and planning. Generate ideas and options, whilst maintaining flexibility and receptiveness to change and alternatives.
- Advanced planning and organisational skills with a demonstrated track record of meeting timelines and adjusting priorities.
- Excellent oral, written and interpersonal communication skills
- Ability to work interdependently and cooperatively as a member of a team
- High self-discipline with the ability to avoid distractions and diversions in order to manage their own time effectively.
