We have an exciting opportunity for an experienced Support Coordinator to join the Civic family on a full-time permanent basis.
ABOUT THE OPPORTUNITY
Our Support Coordinators help our clients to navigate the complexities of the NDIS so that they can access the right supports at the right time. This involves communicating with service providers, organising the supports within clients’ plans, making sure budgets are followed, help with plan reviews and generally making sure that client outcomes are achieved. This role is also responsible for growing referral networks with service providers.

You are someone who wants to go the extra mile to help your clients. You love variety and a fast pace environment. One day you could be organising everything so a person can move into their forever home, the next you could be supporting someone to plan for their yearly review or even assisting a person choose the best allied health professionals to suit their needs. You place them at the center of every decision and make sure they get the final say on which service providers are included and that they feel comfortable and confident with every member of the client’s support team. You make sure you understand your clients’ individual needs and you determinedly resolve issues as quickly as possible. You aren’t just a keyboard warrior and you love to engage directly to get the outcome for your clients. You build great relationships with your key stakeholders (including families) and have good networks with service providers.
Ideally coming in with previous Support Coordination experience, you thrive in a fast-paced work environment and enjoy managing your varied caseload.
If you have been previously working as a Case Manager, an Advocate, a Recovery Coach or a Health Navigator we would love to support you transitioning to this Support Coordinator role.
Typically, the responsibilities are :
- Ensuring connections to services to individual client plans.
- Developing networks within industry to ensure each client meets their goals and aspirations.
- Collaborating with clients to help them better understand the NDIS and maximise their access to support.
- Strengthening client capability and understanding for their improved independence.
- Planning in advance for potential crisis management and advocating for change to promote client safety and wellbeing.
- Assisting clients with plan evaluations, amendments, and renewals.
- Growing Civic’s support coordination networks and referral pipelines.
- Continuing development and evolution of Civic client experience.
ESSENTIAL CRITERIA
You will need to be able to demonstrate the following:
- Previous experience working as a Support Coordinator, ideally within disability, is desirable.
- A demonstrated understanding of the NDIS framework and processes.
- Flexible and adaptable attitude when working with various and diverse stakeholders to achieve a common goal.
- Excellent problem-solving skills and the ability to translate client needs into practical solutions.
- A determination to get things done and a passion for individualised quality care.
- Self-motivated – a consistent creative and proactive approach.
- Ability to work within deadlines and toward performance targets.
- Previous experience in Business Development will be beneficial to this role
- Mental Health qualifications are desirable.
- Previous experience within Social and Affordable Housing is desirable.
