We are currently seeking an experienced Support Coordinator to join our team located at our Head Office in Revesby. Reporting to the Case Manager, the Support Coordinator will be responsible for:
- Delivering quality services and building rapport with NDIS participants and their families
- Supporting participants to understand their plan and supports available to them
- Psychosocial recovery
- Coordinating and facilitating supports and services for participants in order to achieve their goals
- Coaching participants and family members on related matters
- Identifying any risks or barriers and providing solutions
- Build and maintain strong relationships with all stakeholders including government and non-government bodies, service providers, allied health professionals and practitioners.
- Prepare plan review reports
- Creating NDIS plans & assisting participants to manage their NDIS funds.
- Developing recovery-enabling relationships
- Supporting the person with their recovery planning
- Coaching to increase recovery skills and personal capacity, including motivation
- Strengths, resilience and decision-making
- Collaborating with the broader system of supports to ensure supports are recovery oriented
- Supporting engagement with the NDIS, including support with plan implementation
- Documentation and reporting.
The successful candidate will have:
- Demonstrated experience in a similar role with sound understanding of NDIS guidelines and practices - this is essential
- Excellent communication and interpersonal skills
- Ability to problem-solve and overcome complex challenges and barriers
- Driver’s license and access to own vehicle
