- Current Registration with the Occupational Therapy Board of Australia; or Bachelor of Social Work and eligible for membership to the Australian Association of Social Workers; or Current Registration with the Psychology Board of Australia; or Current Registration with the Nurses and Midwives Board of Australia; or current registration with the Physiotherapist Board of Australia
- Demonstrated ability to use problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care.
- Demonstrated effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
- Knowledge and understanding of person-centred care, evidenced based practice and quality improvement.
- Demonstrated understanding of and ability to practice in accordance with legislation, policies and within the scope of practice of your discipline.
- Understanding of the principles of primary health care practice and integrated care and their applicability to service delivery
- Demonstrated understanding of Aged Care Assessment and/or Aged Care service delivery.
- Valid unrestricted Drivers Licence for use in New South Wales/Australia.
