JewishCare is a values driven organisation, we have been providing support to the community for over 85 years. We currently have an exciting opportunity for an NDIS Service Coordinator, to join our small and collaborative team, providing support, information and access to JewishCare's NDIS services for participants in the community.
About the role.
The key function of the NDIS Service Coordinator will be to ensure NDIS participants in the community have access and information in relation to JewishCare NDIS Services. Working with the NDIS Support Workers the Service Coordinator will facilitate all aspects of the delivery of support services, ensuring client centred quality services that meet regulatory requirements.
Key aspects of this role will also include;
- Completing necessary documentation to support JewishCare NDIS services delivery.
- Development of care plans and client profiles, strengthening and enhancing participants capacity to choose the right support for their needs.
- Working with clients and families to ensure goals are being met with NDIS plans.
- Negotiate services to be provided, developing service agreements and creating service bookings.
- Providing guidance and advice to disability support workers, ensuring quality of services provided.
About you.
- Ideally you will have a minimum of a Diploma of Community Services or qualifications in Social Work, Social Science.
- Extensive knowledge of NDIS and Quality and Safeguards,
- Provide e experience in providing expert advice and consultation to NDIS participants and their families on the changing NDIS environment.
- Experience in supporting people with a range of disabilities including intellectual/developmental disability and be passionate about enabling people with intellectual disabilities to live a fulfilling and inclusive life.
- Ability to build NDIS participant and family capacity to understand and navigate service systems.
- Recent experience of working within NDIS environment including pre-planning, planned and unplanned reviews.
- Proven ability to manage multiple priorities within tight time frames and be able to work in a dynamic environment and coordinate a number of activities at one time.
- Excellent customer service skills and proven ability to mediate difficult situations and crises with clients, carers and service providers.
