ROLE REQUIREMENTS :
- Allied health or community services qualification, such as Social Work, Nurse, Counselling, etc (or studying)
- Community services or allied health experience
- Passion for the Disability and Community sector
- Excellent written and verbal communication skills
- Experience in note-taking and report writing
- Creative problem-solving skills
- Excellent organisational and time management skills
- Ability to create and maintain lasting professional relationships
- Caring, empathic, and understanding nature
- Positive and energetic team member
- A strong sense of professional boundaries and integrity
- Brisbane based home to travel from
- Autonomous and preference for working from home
- Current Australian Driver's Licence and access to registered and insured vehicle
PREFERRED BUT NOT ESSENTIAL:
- Understanding of the NDIS and government funding schemes
- Case management or NDIS coordination experience
KEY DUTIES
- Supporting clients to understand and implement the funded supports in their NDIS plans,
- Working collaboratively with clients and their families to build up their capacity to manage the supports within their NDIS plan,
- Linking participants to suitable service providers and allied health professionals,
- Assess client needs and monitor progress toward the achievement of their plan goals,
- Connect with clients in the community and remotely,
- Identify community or mainstream supports that can also assist participants to achieve their set plan goals,
- Communicate with and develop professional relationships with stakeholders, including service providers, allied health professional, NDIA, Queensland Health, Office of the Public Guardian, etc.
